About our Patient Participation Group (PPG)

Would you like to have your say about the services provided at Northdown Surgery? Join our Patient Participation Group and help us shape the future.

See below for answers to some frequently asked questions

We are very proud of our surgery and staff and although we try to do a good job we are always looking at ways to improve our service to our patients.

We have a patient participation group so that you can make suggestions about the way we deliver services to our patients. We would like a wide range of our patients to take part so that the views are representative of the local population.

We would like to email/post to you from time to time to ask your views on questions like opening hours, what other services might be helpful for the surgery to offer or even what you think about the car park and keeping you informed about PPG meetings, minutes etc.

To sign up to become a member of the PPG, please download the form below. After we have received your form you will be added as a member of our Patient Participation Group and will receive  newsletters and links to our surveys so that you can give us your views and PPG minutes and meeting reminders etc. . Please note that no medical information or questions will be responded to.

Many thanks for your assistance - there are two copies of the same form below in different software formats, either of which can be returned to us.
