Please note that it is perfectly acceptable for parents/carers to apply for access on behalf of their children up to the age of 14.  It is also acceptable for parents to apply for access on behalf of children 14 and over, however please note that we will legally need to verify with the adolescent by phone (unless present when the form is presented to Reception) to get consent from the child that they are happy for parents/carers to see their medical record.

If you are between 16-18, you can apply in your own right for access, however please ensure that an email address is held at the surgery for you.

We do appreciate that the 14-18 year old age group does present a challenge when needing to produce ID, particularly photographic ID, however if no driving license or passport is available, photographic evidence can be provided by a student pass, national insurance card, rail card etc.  Similarly it is unlikely that people of this age will be able to produce a household bill with their name on but a copy of any letter, i.e. school, mobile phone company will be accepted.

Although the surgery will endeavour to contact adolescents once they reach 14 to ensure that they are still happy for parents/careers to see their medical record, the responsibility really lies with the individual concerned to contact the surgery to change their email address on the system and to let the surgery know.
