You will need to bring to the surgery a signed consent form (also below) with the 2 necessary ID requirements before access can be processed. Please allow up to 10 working days for this to be processed.

The software company have made some recent changes to the Patient Access system which have affected some patients.
- The main difference is that it is no longer possible to use one email address for several members of the family who have a Patient Access account. This is to adhere to the new GDPR act. The only solution is to create a new email address for each person - don't forget to let the surgery know of your change of email too.
- The second change is (for some reason) it is now necessary to physically put something into the comments box when requesting medications now. If you have no comment, simply just put a full stop in the box. This will then allow you to confirm your medication request.
